Footnotes: Based on Kessner Index criteria, care is deemed inadequate if the mother never received prenatal care, care began in the 3rd trimester, if care included 4 or fewer visits for a pregnancy of 34 or more weeks, 3 or fewer for a pregnancy of 32-33 weeks, 2 or fewer for a pregnancy of 30-31 weeks, one or fewer for a pregnancy of 22-29 weeks, or no visits for a pregnancy of 17-21 weeks.
